Media Consent Form
This consent form gives European football academy Okinawa (EFAO) the permission to use/reuse any photographs and video footage taken during EFAO events for their website and social media channels.
EFAO understands that the use of Social Media helps promote football . The Policy outlines the standards the EFAO requires when using Social Media.
EFAO requires a signed release form from the subject of any photograph or image used on its web site. This form must be completed by individuals or parents of individuals who wish to submit photographs for publication or who may be the subject of photographs to be published on this website. EFAO will not publish any image of a minor unless his or her parent or legal guardian has signed a release form.
